Reporting Illness or Injury

Report all illnesses or injuries, and near misses to the UT System Office of Risk Management. EHS receives notices of these incidents.

Report an Injury/Illness, Incident, or Near-Miss

On-the-Job Injuries (Employee Illness or Injuries)

Employee accidents must follow all Workers Compensation requirements including timely and proper reporting to CorVel. The State of Tennessee can issue substantial fines to your department for failure to report, delayed report, or seeking treatment for non-emergencies prior to reporting.

Employees include: anyone who receives pay directly from UT and is listed in DASH as an active employee, not “Friend of the University”

  • Full and part time staff
  • Faculty
  • Graduate Assistants (GAs, GRAs, GTAs)
  • Student Assistants (employees)
  • Workstudy students

When asked by CorVel: “Who is your employer?’’ Answer with the appropriate response below:

  • ‘I work for the State of TN – UT Knoxville (UTK)
  • ‘I work for the State of TN – UT Graduate School of Medicine
  • ‘I work for the State of TN – UTIA – College of Veterinary Med. (CVM)
  • ‘I work for the State of TN – UT Institute of Agriculture (UTIA)

General Liability (All Near-Misses and Non-employee Illness or Injuries)

Use when an accident involves a non-employee or a near-miss occurs for employees or non employees.

Non-employees include:

  • Visitors
  • Volunteers
  • Friends of the University
  • Students not employed by the University

A near-miss is a specific occurrence where something bad almost happened.

It is an incident in which no property was damaged and no personal injury was sustained, but damage or injury could have occurred, given a slight shift in time or position.
